Required Documentation for Kindergarten Enrollment

Be prepared with the following:

  • Proof of residential address in Lindsay. A utility/rental bill with a current address is acceptable.

  • Proof of birth date

  • Current physical exam record (good for 1 year)

  • Proof of income (preschool and TK ONLY)

  • Immunization record with TB Skin Test

Students Enrolling in Kindergarten REQUIRE the following Immunization Records:

  • Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap, or Td) — 5 doses (4 doses OK if one was given on or after 4th birthday)

  • Polio (OPV or IPV) — 4 doses (3 doses OK if one was given on or after 4th birthday)

  • Hepatitis B — 3 doses
  • Measles, Mumps, and Rubella (MMR) — 2 doses (Both given on or after 1st birthday)
  • Varicella (Chickenpox) — 2 doses

TB (Tuberculosis) Skin Test: Documentation must show the date that the test was given AND results.

To complete registration: It is necessary for you to either scan and upload the record of your child's immunizations OR drop them off at your child's school It is recommended that your child have a health check-up/physical with a doctor or clinic prior to entering kindergarten. A physical will be required by the 90th day of enrollment in first grade. We encourage you to complete this physical during your child's kindergarten year.

Documentacion Requerida Para Kinder

Prepare la siguiente información:

  • Comprobante de domicilio residencial en Lindsay. La factura de servicios públicos o de renta con su domicilio actual es aceptable.

  • Prueba de Nacimiento

  • Examen Fisico (valido por 1 año)

  • Prueba de ingreso (solamente para preescolar y TK)

  • Comprobante de vacunas con pruba de tuberculosis

Los estudiantes que se inscriben en Kinder REQUIEREN los siguientes registros de vacunación:

Prueba cutánea de TB (tuberculosis): La documentación debe mostrar la fecha que la prueba se realizó y los resultados.

Para completar el registro:Es necesario que escanee y cargue el registro de las vacunas de su hijo O lo deje en la escuela de su hijo Se recomienda que su hijo se someta a un chequeo médico / físico con un médico o clínica antes de entrar al jardín de infantes. Se requerirá un examen físico antes de los 90 días de inscripción en primer grado. Le recomendamos que complete este examen físico durante el año de kindergarten de su hijo.
