Please print out a copy of the necessary form, fill it out as accurately and completely as possible, then email the document to the correct individual.  All Facility Request forms should first be sent to the requested site.  Table and Chair Requests, along with Facility Modifications may be emailed to cberger@lindsay.k12.ca.us, at least one week prior to the date needed.  If you have any questions, please email cberger@lindsay.k12.ca.us for assistance.
